Job Summary
As a Social Services Case Manager, you will work with individuals and families to help them find the support they need. This includes talking to clients to understand their needs, creating a plan to help them, and connecting them with services like health care, food programs, and housing help.
You will be a key part of making sure people in your community get the support they need to live better lives. You’ll help them feel cared for and guide them through important services that can change their lives for the better.
Key Responsibilities
As a Social Services Case Manager, your job will include a variety of important tasks. Here are some of the things you will do:
1. Talk to Clients and Understand Their Needs
You will meet with people who need help. You’ll listen to their stories, learn about their situation, and figure out what kind of help they need.
2. Create a Plan to Help Each Client
Once you know what help the person needs, you will create a plan that shows what services and support they should get. This could include help with food, health care, housing, or other services.
3. Help Clients Connect with Services
You will help clients reach out to different organizations and programs that can provide the support they need. You’ll make sure they know how to apply for these services.
4. Check In with Clients to See How They’re Doing
You will follow up with clients to make sure they are getting the help they need. If there are any problems, you will work with them to fix it.
5. Be a Supportive Friend and Guide
You will listen to clients, offer advice, and make them feel comfortable. You’ll be there to help them understand the steps they need to take to improve their lives.
6. Keep Records
You will keep track of each person’s progress by writing down notes about your meetings and what services they are using. It’s important to stay organized so everyone gets the help they need.
Skills You’ll Need
To do a great job as a Social Services Case Manager, you will need the following skills:
Required Skills:
- Great Communication: You should be able to talk clearly and listen carefully to understand what clients need.
- Problem-Solving: Sometimes things don’t go as planned. You’ll need to be able to think of new solutions to help clients.
- Kindness and Patience: Many people you help may be going through tough times. Being kind and patient will help them feel more comfortable.
- Organization: You’ll be helping many people at once, so being able to keep track of all the details is important.
- Teamwork: You will work with other people who provide services to clients, so being a good team player is important.
Preferred Skills (Nice to Have):
- Experience Helping People: If you have worked with people before or helped others in your community, that’s a plus!
- Bilingual: If you speak more than one language, especially Spanish, it can help you work with more people.
- Knowledge of Social Services: If you know about programs like food stamps, health care, or housing support, that will be very helpful.
Education and Experience
To be a Social Services Case Manager, here’s what you need:
- Education: You must have a high school diploma or a GED. If you have a college degree in a related field, like Social Work or Psychology, that’s even better, but not required.
- Experience: It’s helpful if you have worked in a job where you helped others before, but we’re also happy to train you if you don’t have experience yet.
- Other Certifications: If you have any special training or certifications related to social work or case management, let us know!
Location
This job is based in San Diego County, CA. You will work in an office, but you may also visit people in their homes or at community centers to help them directly. San Diego is a beautiful place with lots of sunshine, parks, and fun things to do, so you’ll enjoy living and working here!
