Service Linkage Worker, HIV Full Time

Public Health Services is looking for someone kind and caring to help people in our community. Job Summary

As a Service Linkage Worker, your job is to help people with HIV find the support they need. You’ll connect them with healthcare, housing, and other services, and help them stay on track with their care. You’ll work closely with clients, families, doctors, and community groups to make sure everyone gets the help they need. This job is perfect if you enjoy helping others and want to bring a positive impact to your community.

What You’ll Do

Helping People Find the Right Support

• Share Information: Help clients understand the health services available to them.
• Connect People to Services: Help people access the healthcare, housing, and support they need.
• Stay in Touch: Follow up with clients to make sure they’re getting the support they need.

Working with the Community

• Work with Doctors: Team up with healthcare providers to make sure clients get the best care.
• Connect with Community Groups: Partner with local organizations that help people with HIV.
• Join Community Events: Help with community outreach events to meet more people who need services.

Being a Supportive Friend

• Listen and Support: Be there for clients and their families with a kind ear and helpful advice.
• Teach Healthy Choices: Educate clients on healthy lifestyle tips and available resources.
• Stand Up for Clients: Help make sure clients’ needs are heard and met by healthcare providers and social services.

What You Need to Have

Must-Have Skills

• Good Communication: Able to talk clearly and explain things in simple ways.
• Kindness and Care: Friendly and understanding attitude toward clients.
• Problem-Solving Skills: Ability to find solutions for clients who may face different challenges.
• Organization Skills: Able to manage different tasks and remember details.
• Basic HIV Knowledge: Understand what HIV is, how it impacts people, and what care options are available.

Nice-to-Have Skills

• Bilingual: Knowing Spanish in addition to English is helpful.
• Social Services Experience: Experience working in healthcare or with community services.
• Knowledge of Local Services: Familiarity with Houston and Harris County’s health services for HIV support.

Education and Experience

• High School Diploma or GED is required.
• Some College Courses in health, social work, or a similar field are preferred.
• Experience: 1-2 years in a social service or healthcare role is helpful, especially if you have experience with HIV-related services.
