Job Summary
In this job, you will be part of a team that brings healthcare services directly to neighborhoods in Harris County. You will help people who need medical care by doing check-ups, giving advice, and treating common health problems. Your work will help ensure that everyone can get the care they need, no matter where they live.
Key Responsibilities
As a Mid-Level Provider, you will have important tasks to do. Here are some of your main responsibilities:
1. Taking Care of Patients
- Check-Up: You will meet with patients, ask them questions, and check their health.
- Diagnosing: You will figure out what is wrong with patients and how to help them feel better.
- Treatment Plans: You will create plans to help patients get well and feel better.
2. Educating Patients
- Health Advice: You will teach patients how to stay healthy and manage any ongoing health issues.
- Lifestyle Tips: You will give tips on eating healthy, exercising, and taking care of their mental health.
- Answer Questions: You will listen to patients’ questions and help them understand their health.
3. Working with a Team
- Collaboration: You will work with doctors, nurses, and other healthcare workers to provide the best care.
- Community Support: You will connect patients with community resources to help them get more support.
- Documentation: You will keep notes about patients’ visits and treatments.
4. Administrative Duties
- Patient Records: You will make sure patient records are accurate and safe.
- Training and Meetings: You will attend training sessions and team meetings to learn new things and improve your skills.
- Outreach: You will help promote our health services to people in the community.
Qualifications
To do this job well, you need to have certain skills and qualifications. Here’s what we are looking for:
Required Skills
- Patient Care Skills: You should know how to examine patients and diagnose common health problems.
- Communication Skills: You need to be able to talk to patients clearly and listen to their concerns.
- Teaching Skills: You should be able to explain health information in a way that’s easy for patients to understand.
- Organizational Skills: You need to manage your time well and keep records organized.
Preferred Skills
- Bilingual Skills: If you can speak Spanish or other languages, that would be helpful.
- Mobile Health Experience: Experience in mobile healthcare is a plus.
- Cultural Awareness: Understanding different cultures in our community is important.
Education and Experience
To apply for this job, you need to meet these requirements:
- Education: You should have a master’s degree from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program.
- Licensure: You must have a valid license to practice as an NP or PA in Texas.
- Experience: Having at least two years of experience as an NP or PA is preferred.
- Certifications: You should have Basic Life Support (BLS) certification. Advanced Cardiovascular Life Support (ACLS) certification is a plus.
Location
This job is based in Harris County, Houston, Texas. You will travel to different neighborhoods to provide care to the community.
