We are looking for a Community Health Worker (CHW) to work from home in Virginia. As a CHW, you will help Medicaid members get the care they need. You will also talk to them about their health and explain how they can use their health benefits. You will make sure they know about things like food, housing, and transportation, which can affect their health. This job is mostly about talking to people on the phone and helping them with their health care.
Job Details:
- Position: Community Health Worker (Remote)
- Location: Remote (Must live in Virginia)
- Job Type: Full-time
- Schedule: Monday to Friday; 8-hour or 10-hour shifts (hours between 7 am and 7 pm)
- Overtime: Available
- Medical Specialty: Public Health
- Shift: Day shift, no nights, weekends, or holidays
Primary Responsibilities:
As a Community Health Worker, you will have the following tasks:
- Talk to New Members: Call new members and welcome them. Help them understand their health care needs by completing tasks like medical screenings and scheduling appointments.
- Build Relationships: Make sure members have a good relationship with their doctors. Help them visit their doctor regularly and stay out of the hospital.
- Help Members with Health Care: Teach members about how to use their health benefits and answer their questions.
- Work with Other Care Providers: Team up with doctors, social workers, and other helpers to make sure the member gets the best care.
- Help with Life Challenges: Assist members with things that affect their health, like food, housing, and getting to medical appointments.
- Keep in Touch with Members: Reach out to people who haven’t been in touch with the health plan for a while. Help them reconnect and get the support they need.
- Learn About Communities: Understand the cultures and values of the community to communicate with people in the best way.
Qualifications:
Required:
- Education: You need a High School Diploma or GED (or higher).
- Experience:
- At least 1 year of experience helping people in the community. You should know about local resources and the culture of the area.
- At least 1 year of experience in a health care setting.
- You should be familiar with Microsoft Office programs like Word, Excel, and Outlook.
- Location: You must live in Virginia to apply for this job.
- Work Requirements:
- Be ready to work regular hours with some overtime.
- Have access to high-speed internet at home and a quiet space to work.
- You need a private workspace where you can protect health information.
Preferred:
- Certifications: It is helpful if you have certifications like a Certified Nursing Assistant (CNA), Home Health Aide, or Medical Assistant.
- Training: A certification as a Community Health Worker (CHW) is a plus.
- Experience:
- Experience with managed care or understanding of the VA Medicaid population is helpful.
- Experience working in jobs where you need to meet goals and stay productive is a bonus.
- Being bilingual (Spanish and English) is very helpful.
- Telecommuting: You will need to follow UnitedHealth Group’s rules for working from home.
