As a Care Navigator, you will help our members get the care they need, whether it’s seeing a doctor, going to the hospital, or finding a specialist. You will work with patients to make sure they know what to do and help them understand their healthcare options.
This job is a full-time remote position, meaning you can work from home. You will work Monday through Friday, from 8:00 AM to 6:00 PM EST. If you enjoy working with people and organizing tasks, this could be a great opportunity for you.

What You Will Do
As a Care Navigator, you will help patients in many important ways. Your job will be to make sure patients get the care they need without any problems or delays. Here’s what you will do:
- Help patients find the right care: You will talk to patients and understand what type of care they need. Then, you’ll help them find the best doctors or specialists for their health problems.
- Coordinate doctor visits: If a patient needs to see a doctor outside of Sollis Health, you will help them get an appointment with the right specialist.
- Organize care services: Sometimes patients need extra help like private nursing or hospice care. You will help arrange these services for them.
- Work with doctors and nurses: You will talk to doctors, nurses, and other healthcare workers to make sure everyone is working together to help the patient.
- Solve problems: If there is a delay or problem with a patient’s care, you will work to fix it so that the patient doesn’t have to wait.
- Find new doctors or services: If a patient needs a new doctor or special service, you will research and find the best options for them.
- Keep patient records updated: You will keep patient information up-to-date in our systems like Athena Health and Salesforce.
- Send medical records: You may need to send a patient’s medical records to their doctor or another provider.
- Provide excellent customer service: You will make sure patients feel heard, supported, and cared for every time they reach out for help.
- Suggest improvements: You will work with other teams at Sollis Health to find ways to make the care process better and smoother for everyone.
What You Need to Know
To do well in this job, you will need to have the following skills and experience:
- Education: You need to have at least an associate’s degree. A bachelor’s degree is even better.
- Experience: You need at least 2 years of experience in an office or healthcare-related job. If you’ve worked in urgent care, a doctor’s office, or a hospital, that’s a big plus.
- Tech skills: You should be comfortable using computers, including CRM systems, health records systems, or other databases.
- Organizational skills: You must be good at keeping things organized and managing multiple tasks at once.
- Communication skills: You need to be able to talk and write clearly. You will work with many people, including patients and doctors, so clear communication is very important.
- Independence: You should be able to work on your own and make decisions when needed.
- Strong work ethic: Being responsible and having a good attitude is key to succeeding in this job.
Pay and Benefits
The pay for this position is $25 – $35 per hour. This pay is for candidates in New York, but if you are located in another state, the pay may be different. Your actual pay will depend on your experience, where you live, and other factors.
