About the Job
Do you want to help people who need special care? The Health & Human Services Agency (HHSA) in San Diego County is looking for someone to join our team as an Integrated Care Coordinator. If you are kind, organized, and enjoy helping others, this job could be perfect for you!
In this role, you will help people who need many different types of care, like health care, mental health support, and other services. You will work with doctors, nurses, social workers, and other helpers to make sure people get the care they need.
Job Summary
As an Integrated Care Coordinator, your job is to help people who need different kinds of care to get it all in one place. This can be people who need medical care, mental health help, or support with daily life. You will make sure they have a plan to get all the services they need, and you will help them understand how to use those services.
Your goal will be to make sure the people you work with stay healthy and get the help they need to live their best lives.
What You’ll Do
Here are some of the things you will do in this role:
1. Help People with Their Care Plans
You will talk to clients (the people you help) to find out what kind of care they need. You will make a plan for them to get all the services they need, like health care, mental health care, or other support.
2. Make Sure Care is Organized
You will work with doctors, nurses, and other helpers to make sure everyone is doing their part. You will check that the client gets all their appointments and treatments.
3. Support Clients
Sometimes, people don’t know how to get the help they need. You will help them find the right services and explain how to use them. You will also make sure clients understand what is happening with their care and answer their questions.
4. Keep Track of Progress
You will check in with clients to see how they are doing with their care. If they need more help, you will adjust their plan to make sure they get the best care possible.
5. Work as a Team
You will work with other people who help the clients, like doctors, nurses, social workers, and other helpers. Everyone will work together to give the best care possible.
6. Keep Good Records
You will keep track of everything that happens with the client’s care. You will write down when they have appointments, what kind of care they received, and what needs to happen next. This helps everyone stay on track.
7. Solve Problems
Sometimes, there will be problems that keep clients from getting care. You will work to find solutions and make sure nothing gets in the way of their care.
8. Learn and Grow
You will have chances to learn more about your job and how to help people even better. The agency supports your growth and learning.

Here are some skills that will help you succeed in this job:
Required Skills:
- Good at Talking to People: You need to be able to talk clearly with clients, their families, and doctors. You should be able to explain things simply and listen carefully to others.
- Organized: You will have many clients to help, so you must keep track of their appointments and care plans. You should be able to stay on top of everything without forgetting important details.
- Problem-Solver: Sometimes things don’t go as planned. You will need to find solutions and make sure clients get the care they need.
- Kind and Helpful: You should be friendly, caring, and always ready to help others. Clients will depend on you to help them through difficult times.
- Team Player: You will work with a lot of other people to help clients, so you must be a good team member and work well with others.
- Attention to Detail: You need to be careful when you write down information, keep records, and track progress. Accuracy is very important in this job.
Preferred Skills:
- Experience Helping People: If you have worked in a job where you helped people or organized things, it will help you in this role.
- Technology Skills: If you have used computers or software for keeping records, that’s a big plus.
- Speak Other Languages: If you can speak Spanish or another language, it will help you work with more people.
- Experience with Mental Health: If you have worked with people who need mental health support, it will be helpful.
Education and Experience
To be considered for this job, you need the following:
- Education: A high school diploma is required. It’s even better if you have some college education or a degree in something like social work, health care, or something related.
- Experience: You need at least one year of experience helping people or working in a job where you organized care or services. If you’ve worked in a hospital, clinic, or any job that involves helping people, that’s a plus!
Where is This Job?
This job is located in San Diego County, California. You may need to travel to different places in the county, like homes, doctors’ offices, and community centers, to meet with clients and help them get the care they need.
