What You Will Do
Support Mental Health Recovery
- Talk with clients to understand their challenges and mental health needs
- Help create care plans that match their goals and personal situation
- Encourage clients to stay involved in treatment and take steps toward recovery
Connect People with Resources
- Guide clients to housing, healthcare, job support, or financial assistance
- Work with community organizations and service providers
- Make sure clients are getting the right support at the right time
Track Progress and Follow Up
- Monitor client progress and adjust care plans as needed
- Maintain accurate records of services, plans, and outcomes
- Follow up to ensure clients are continuing to receive the help they need
Collaborate with Healthcare Teams
- Partner with therapists, doctors, and social workers to deliver quality care
- Share updates and insights to improve client support
- Respond to crises and help connect clients to emergency care when necessary

Required Qualifications
To be a successful candidate, you should have:
- A Bachelor’s degree in social work, psychology, counseling, or a related field
- At least 2 years of experience in mental health, case management, or social services
- A valid California driver’s license
- Strong communication, organizational, and problem-solving skills
- A caring, patient, and compassionate approach to client care
Preferred Qualifications
- A Master’s degree in a behavioral health field
- Licensure as an LCSW, LMFT, or LPCC
- Experience supporting people during mental health crises
- Familiarity with community mental health programs in San Diego
- Ability to speak Spanish or other commonly spoken languages
