Job Summary
As a Cohort Manager, your job will be to help manage programs that focus on chronic diseases affecting the Latino community. Chronic diseases are long-term health problems, like diabetes and high blood pressure. You will work with people in the community to help them understand these diseases and learn how to stay healthy. Your main job is to connect with people, organize events, and provide support to families.
Responsibilities
In this job, you will have many important tasks, including:
- Creating Programs: You will design and run health programs that help Latino families learn about chronic diseases and how to prevent them.
- Talking to the Community: You will meet with community members, local organizations, and health workers to build strong relationships and teamwork.
- Teaching: You will hold workshops and meetings to teach people about staying healthy and preventing diseases.
- Collecting Information: You will gather information about how well the programs are working and write reports about them.
- Leading a Team: You will manage a team of health workers and volunteers to make sure everyone is doing their part in helping the community.
- Managing Resources: You will keep track of the money and supplies needed for the health programs.
- Being an Advocate: You will speak up for the health needs of the Latino community and share their concerns with health officials and other leaders.
Qualifications

- Good Communication: You need to be able to talk and write clearly so you can help people understand important information.
- Strong Leadership: You should be able to lead and inspire your team and community members.
- Organizational Skills: You will need to keep track of many tasks and details.
- Cultural Understanding: You should respect and understand different cultures and how they can affect health.
- Problem-Solving Skills: You need to think of smart solutions to challenges that come up.
Preferred Skills
- Bilingual: It’s great if you can speak both English and Spanish to reach more people in the community.
- Experience in Public Health: If you have worked in public health or with chronic diseases before, that would be helpful.
- Grant Writing: Knowing how to write grants to get funding for programs is a plus.
Education and Experience
- Education: You need to have a bachelor’s degree in public health, social work, or a similar field. A master’s degree is even better!
- Experience: You should have at least three years of experience working in community health or program management.
